P0138 indicates the downstream (post-catalytic converter) oxygen sensor on Bank 1 is reporting a consistently high voltage above normal range, typically above 0.9 volts. This sensor should normally show low, fluctuating voltage after exhaust gases have passed through the catalytic converter. A sustained high voltage suggests the sensor is detecting persistently rich exhaust or has failed internally.

Symptoms

What causes it — most likely first

1. Failed downstream O2 sensor (most likely)

The sensor itself has degraded internal circuitry, contaminated heating element, or failed reference electrode causing it to read high voltage even when exhaust is not rich.

2. Exhaust leak after the sensor

A leak in the exhaust pipe or muffler downstream of the sensor can allow outside air to enter, causing false high voltage readings.

3. Rich fuel condition

A leaking fuel injector, faulty fuel pressure regulator, or evap system leak can cause overly rich air-fuel mixture that the post-cat sensor detects as high voltage.

4. Wiring/connector issue

Damaged wiring, corroded connector, or short-to-voltage in the O2 sensor circuit can cause artificially high sensor readings at the PCM.

5. Coolant leak into cylinders

Failed head gasket allowing coolant to enter combustion chambers creates rich running conditions detected by the sensor.

6. PCM/ECM fault (rare, last resort)

The engine computer may have a fault in its input circuit for this sensor, but this is infrequently the cause and should be diagnosed only after all other possibilities are ruled out.

How it's diagnosed

  1. Connect OBD-II scanner, record freeze frame data and confirm P0138 is present with the same failure pattern on multiple key-on cycles
  2. Perform visual inspection of the Bank 1 Sensor 2 O2 sensor, connector, and wiring for damage, corrosion, or melt damage
  3. Monitor the downstream O2 sensor voltage with a scan tool or multimeter at idle; a properly functioning post-cat sensor should read steadily between 0.1-0.5V and show minimal fluctuation
  4. Check for exhaust leaks by visual inspection and smoke test downstream of the sensor
  5. Test fuel system pressure and inspect fuel injectors for leaks or excessive fuel trim values on the upstream sensor
  6. If all above tests pass, consider PCM input circuit fault only as a final排除 diagnosis
