P2195 indicates that the powertrain control module has detected the Bank 1 Sensor 1 oxygen sensor signal is biased or stuck in a lean condition. This is the upstream oxygen sensor on bank 1, located before the catalytic converter. The PCM sees the sensor reporting lean (low voltage) for longer than expected and cannot correct for it through fuel trims.

1. Failed Bank 1 Sensor 1 oxygen sensor

The sensor itself has degraded internally and is no longer producing an accurate voltage signal, causing it to report lean regardless of actual exhaust oxygen content. This is the most common cause.

2. Exhaust leak upstream of the sensor

A crack, leaking gasket, or loose connection in the exhaust manifold or pipe before the sensor allows outside air to enter the stream, which the sensor reads as a lean condition.

3. Engine vacuum or intake air leak

Unmetered air entering the intake after the MAF sensor — from a leaking vacuum hose, intake manifold gasket, or PCV system — creates a genuinely lean mixture that the sensor correctly reports.

4. Fuel delivery problem causing lean condition

Low fuel pressure, a clogged or failing fuel injector on bank 1, or a weak fuel pump can result in an actual lean condition that the oxygen sensor is correctly detecting.

5. Wiring or connector fault on sensor circuit

Corroded, chafed, or loose wiring and connector terminals at the oxygen sensor can alter the voltage signal reaching the PCM, causing a false lean reading.

6. Faulty PCM (rare)

In uncommon cases, an internal failure in the PCM's sensor input circuit may misinterpret a valid sensor signal as lean. This is the least likely cause and should only be considered after all other possibilities are eliminated.

How it's diagnosed

  1. Read and record all stored codes and freeze-frame data to confirm P2195 and check for related lean or fuel-system codes.
  2. Inspect the oxygen sensor wiring and connector for damage, corrosion, or loose pins, and repair as needed before further testing.
  3. Check for exhaust leaks upstream of the Bank 1 Sensor 1 location, including manifold gaskets and exhaust connections.
  4. Use a scan tool to observe live data: compare Bank 1 Sensor 1 voltage response to Bank 2 Sensor 1 if available, and review fuel trim values to determine if a genuine lean condition exists.
  5. Perform a vacuum leak and intake leak inspection using smoke testing or propane method if fuel trims are high positive.
  6. Verify fuel pressure and injector performance on bank 1 if a real lean condition is confirmed and no vacuum or exhaust leak is found.
